Creating Attachment Question
An Attachment type of Checklist Question allows a technician to upload images and videos in the Checklist during the inspection.
To create an Attachment Question:
1. Go to ServiceMax Setup Checklist Manager Checklist Designer. The Checklist List view is displayed.
2. Click an existing checklist to open it in edit mode. The Checklist Form Designer page is displayed.
3. Click the Add New Questionlink to insert a new question. The Add Question dialog box is displayed.
4. Set the Question Type as Attachment and enter the information for the fields like Question text, Help URL, and Tags in the Basic Information tab.
5. Click Done and the question is created.
Basic Information Tab
The Basic Info tab allows you to select the type of question and provide the Question Name, Tags, and Help URL.
Enter the details for the fields as described in the following table:

Question Type | This field allows you to select a question type from the list. |
Question Text | This field allows you to define the question in this field. This field is editable. |
Required | This field indicates whether the question is optional or mandatory to answer. |
Help URL | This field is the placeholder for the help URL of the question. (Not mandatory) |
Tags | This field categorizes the questions under multiple tags. (Not mandatory) |
Include in Output Document | This field is not mandatory. On enabling this field, the attached file is printed in the Output Document. |
Response Tab
The Response tab allows you to set a default response.
Description Required: This field allows you to provide a description for the attached file.
Validation Tab
The Validation tab allows you to add data validation rules and error messages.
Validation is not applicable for the Attachment Question type.
