Form Design File Overview
A Form Design file determines the appearance of a form on which you record data in the System file. The default Form Design files that are created along with the Project cannot be modified. Consequently, unless you create a custom form, you cannot open a Form Design file.
Because viewing a completed Form Design file makes it easier to understand how to create one, a custom FRACAS Incident Entry form is part of the Tablet PC Sample Project, which is in the Samples folder on the Windchill Risk and Reliability application server. When you create a Form Design file, you select the table with which to associate the file. For more information, see Table Type Selection. The following table types support the creation of Form Design files:

Once the Form Design file is created, you insert fields from the selected table and any related tables using either the Form Design Wizard or the Form Designer.

In the Options window, Report and Form Designers > Use Form Design Wizard controls whether the Form Design Wizard or Form Designer starts when you create or open a Form Design file. When this checkbox is selected (default), the wizard starts. When it is cleared, the Form Designer starts. For more information, see Report and Form Designers User Options.
All data fields and objects that you insert in a Form Design file are known as controls. Comprehensive information about inserting controls appears in Insert Control Commands. If you insert fields from related tables, they are display-only.
The controls that you insert in the Form Design file are ordered from back to front as follows:
Picture controls
Group boxes
All other controls, according to their ascending tab order
