A design in is a top-level folder containing folders, sheets and catalogs. Catalogs are visible when you are working with the Catalog Explorer.

Folders contain links to other folders and sheets in the design hierarchy. A new folder is linked automatically to the folder currently selected. You can link it to more than one folder. Sheets are created in the folder currently selected and are also linked to the folders. You can cut and paste folders into other folders.
