Control Plan/DVP Header Fields

The following table describes the data fields that are shown by default in the FMEA Control Plan Header window and FMEA DVP Header window. Only the first field differs. In the FMEA Control Plan Header window, the first field is Control Plan Number. In the FMEA DVP Header window, the first field is DVP Number.

Field	Description
Control Plan Number OR DVP Number	The number assigned to the control plan or DVP. If suppliers have their own control plan or DVP in place, they might have their own numbering system.
First Issue Date	The date that the control plan or DVP was made available.
Part Number	The specific part number that the control plan or DVP addresses.
Revision Date	The date that the control plan or DVP was last changed.
Part Description	A detailed description of the part being addressed.
Core Team	The names or initials of everyone involved with writing the control plan or DVP.
Key Contact	The name and telephone number of the primary contact responsible for the control plan or DVP.
Supplier Contact	The name and telephone number of the primary contact for the supplier.
Supplier/Plant	The name, corporate, and plant location of the supplier.
Supplier Code	The code used to uniquely identify the supplier.
Customer Engineering Approval	The name of the customer engineering representative who reviewed and approved the control plan or DVP.
Customer Engineering Approval Date	The date that the control plan or DVP was approved by customer engineering.
Customer Quality Approval	The name of the customer quality representative who reviewed and approved the control plan or DVP.
Customer Quality Approval Date	The date that the control plan or DVP was approved by customer quality.
Supplier Approval	The name of the supplier representative who reviewed and approved the control plan or DVP.
Supplier Approval Date	The date that the control plan or DVP was approved by the supplier.
Other Approval	The name of any other representative who reviewed and approved the control plan or DVP.
Other Approval Date	The date that the control plan or DVP was approved by any other representative.
