FMEA Mode Record Creation from FRACAS Incident Records
As described in Building a FMEA from FRACAS Incidents, you use the Build FMEA from FRACAS Wizard to create FMEA mode records from FRACAS incident records. A FMEA mode record is created under the same system tree item as the FRACAS incident record from which it came. One FMEA item is created for all modes created under a system tree item. Failure Mode is the key field for determining duplicate modes.
For example, perhaps three incidents are really the same failure mode. Because they are the same failure mode, each of these incidents has the same data in one of their fields. Assume that it is Defect Category. You would map Defect Category in the FRACAS module to Failure Mode in the FMEA module.
Each time the same data is found in Defect Category under the same system tree item, it is considered to be the same mode. Each of the incidents in the group creates a new cause or effect record under the mode if any data is mapped to cause or effect fields.
If no data is mapped to cause and effect fields:
The Mode value remains the same as the first incident found from all those in the group.
The Mode Percentage value for a new record is equal to the number of contributing incidents divided by the total number of modes for this system tree item.
For example, assume that five incidents are inserted for one system tree item. Three of these items are grouped, and two of them are not. The group of three incident modes is assigned a Mode Percentage value of 60. Each of the other two individual incident modes is assigned a Mode Percentage value of 20. This would hold true even if no cause and effect records were created for the duplicate modes.
If you do not pick a FRACAS field with the same data across multiple incidents as the key field, then every incident becomes its own mode. An option is provided to group the same items that appear multiple times in the system tree. When you step through the Build FMEA from FRACAS Wizard, on the Common Field page, you select a field in the System Tree Items table to use to determine which system tree items are the same. The default is Part Number. During the FMEA creation process, any incident under one system tree item creates FMEA records under all system tree items that have the same data in the selected field.
