FMEA Mode Table BI
The following table describes the data that appears in characters 36 through 62 of the FMEA Mode Table BI. This table is outputted for each failure mode in the FMEA Worksheet table.

FAILURE MODE INDICATOR
Always set to F.

MODE NUMBER
If the Mode number field in the Generate LSAR File window is set to < Automatically generated > , the modes are numbered from 1 to n for each mode associated with an item. If a field from the FMEA Worksheet table is selected, the first three characters in this field are used. A common numbering scheme is 1A1, where:
The first character represents the item
The second character represents each unique failure mode for each item
The third character represents the cause of failure for each failure mode

MISSION PHASE CODE
All phases in the Project’s Mission Profile file are numbered sequentially. The selection for the Mission Phase field for the mode is converted to the number of the association phase from the Mission Profile file. This number is used as the mission phase code. If no phase is selected for the Mission Phase field, then an X is used to indicate that the failure mode applies to all phases.

SAFETY HAZARD SEVERITY CODE
In the FMEA Worksheet table, the Severity field must be at the mode level and tied to a list. This code is the value associated with the selected list item. The value must be 1, 2, 3, or 4. Otherwise, a 0 is used as the code.

FAILURE EFFECT PROBABILITY
Displays the value for the Failure Effect Probability field at the mode level in the FMEA Worksheet table.

FAILURE MODE CRITICALITY NUMBER
Displays the value for the Mode Criticality field at the mode level in the FMEA Worksheet table.

FAILURE PROBABILITY LEVEL
In the FMEA Worksheet table, the Occurrence field must be at the mode level and tied to a list. A value is obtained from the value associated with the selected list item. The value must be 0, 1, 2, 3, or 4. The code is created by converting the value to a letter as follows: 0 = A, 1 = B, 2 = sC, and so on. Otherwise, a blank space is used as the code.

Displays the value for the Mode Operating Time field at the mode level in the FMEA Worksheet table.
