Creating Failure Modes
To add a failure mode, you must have permission to create failure mode objects in your quality context. This permission is assigned to the Quality System Manager and Reliability Engineer roles out-of-the-box.

You can have read-only permissions for the context, but still have permission to create failure mode objects. In this case, you can see the failure mode name, but the context appears as (Secured Information) and you cannot see the actual name of the context.
To create a predicted failure mode, do the following:
1. In your quality context, click Failure Modes.
2. Click the create predicted failure modes icon .
3. In the Create Failure Mode window, complete the following fields:
Field
Description
Name
Required. Enter a name for the failure mode.
Number
The system automatically generates a number for the failure mode. The number provides another way for users to search for a failure mode.
Tags
Select a tag for the failure mode. Click the plus sign to add more than one tag to the failure mode.
Use tags to group failure codes for easier filtering and searching.
To create additional tags for failure modes, users with the correct permissions can add them as an enumerated list. For more information, see Enumerated Value List Constraints.
4. Click OK.
