Things to Avoid
You code an entry to have either a see reference or to have a page number, not both. If code for both exists, the reference will not appear in the index. This coding is shown in the example below. The first line is coding for a page number. The other two lines are for a see reference.

You code either a see or see also reference for an element, not both. If code for both exists, the reference will not appear in the index. This coding is shown in the example below.
