<?tbmct>
Description
This command allows you to specify an alternative table top margin which is only used when the table occurs at the top of a column. This margin will only occur at the start of the table and will not apply for mid-table rows that appear at the top of a column after the table has overflowed into a new column.

The margin set with this command (even if its value is 0) will override any set with a <?tbmt> command. To default to using the standard top margin in all circumstances, specify <?tbmct=t> for the column top margin.
You may specify a priority by which this margin is judged if margins need to be merged to save space — use the <?tbmctp> command.
Syntax
<?tbmct height:uh>
height
The height of the margin, with optional decimal part and optional unit of measurement.
